Vincent Giolito is a senior researcher with the Baillet Latour chair on error management at Solvay Brussels School of Economics and Management (BSEM). He was a leadership coach in Paris for the past six years. Previously, he was a business reporter and editor in the French media for more than 15 years. He was the business pages editor of the French newspaper Le Figaro.

Jan U. Hagen is an associate professor at European School of Management and Technology (ESMT) Berlin and author of Confronting Mistakes—Lessons from the Aviation Industry When Dealing with Mistakes. His recent work focuses on error management, team interaction, and leadership in high-reliability organizations such as aviation, health care, and the military.

David A. Hofmann is the Hugh L. McColl, Jr. Distinguished Professor of Leadership and Organizational Behavior at University of North Carolina. He is an award-winning researcher and teacher in organizational behavior and leadership. His research played a key role in the organizational changes instituted at NASA after the Columbia accident.

Peter Klement is Director of Aviation Safety of the German Federal Armed Forces in the rank of Brigadier General. During his career, he flew 3000 hours in fighter aircraft and has held numerous leadership and staff positions, during which he actively promoted aviation safety and flight training programs. In his current position, he is responsible for Accident and Incident Investigation, Accident Prevention, and Aviation Safety Training programs of the German Armed Forces.

L. David Marquet is a former nuclear submarine commander of the US Navy and author of Turn the Ship Around!: A True Story of Turning Followers into Leaders.

Mark Mellett is Vice Admiral and Chief of Defence, Irish Defence Forces. He has over 39 years’ service as an officer in the Irish Defence Forces.  He is the first naval officer in the history of the Irish State to serve as Chief of Defence, having previously served as Deputy Chief of Defence and Chief of Navy. Admiral Mellett has extensive experience at home and abroad, including in Afghanistan and Lebanon, in combined and joint operational missions. Admiral Mellett is a champion of internal and external organisational diversity.  He is a strong advocate of external networks and partnership with other militaries, state bodies, research institutes and enterprise.

Julianne Morath is the president and CEO of the Hospital Quality Institute (HQI), a collaboration of the California Hospital Association and three regional associations. She is a founding and current member of the NPSF Lucian Leape Institute. As the author of two books, The Quality Advantage and To Do No Harm, she has published widely on the topics of quality, patient safety, leadership, and patient/family engagement.

Nathalie Pattyn is a Major with the Belgian Armed Forces and an Associate Professor in Biological Psychology at the Vrije Universiteit Brussel and in Human Performance at the Royal Military Academy. Her research interests include the psychophysiological measures of performance, sleep in extreme environments, and Human Factors approach to aviation and other high performance environments. She has a mixed clinical, research and operational background, having been deployed as a medical officer in Afghanistan and Antarctica, currently still working as an emergency physician and a flight surgeon.

Rangaraj Ramanujam is the Richard M. and Betty Ruth Miller Professor of Management at Vanderbilt University. He is a leading researcher and consultant on the organizational causes and consequences of operational failures in high-risk work settings. His current research examines the role of leadership, communication, and learning processes in enhancing the quality and safety of healthcare.

Karlene Roberts is professor emeritus at Berkeley Haas School of Business, University of California Berkeley, USA and a pioneer in research on high-reliability organizations. The results of her research have been applied to U.S. Navy aircraft carrier operations, the U.S. Coast Guard, The Society for Worldwide Interbank Financial Telecommunication (SWIFT), U.S. Federal Aviation’s Air Traffic Control System NASA.

Katsuhiko Shimizu is professor of organizational management at the Business School of Keio University in Tokyo, Japan. His research focuses on Strategic flexibility, that is, managerial challenges under uncertainty; organizational learning from mistakes and, more broadly, strategy implementation. He authored a number of articles about errors and error management from the perspective of top executives, including mistakes made at the top of organizations.

Paul J. Verdin is the director of the Baillet Latour Chair on Error Management at Solvay Brussels School of Economics and Management, where he also holds the Chair in Strategy and Organization. He is a visiting professor at the Berlin School of Creative Leadership and Deusto Business School and was a former full-time faculty member at IESE, INSEAD, KU Leuven, and Tilburg University.
